This request for information is made pursuant to the Freedom of Information Act, 5 USC 552 & its promulgated regulations. I would appreciate a response in 15 days.
This is a promise to pay fees for duplicating the documents & information requested herein. Please obtain approval from me before incurring any expenses in excess of $50.00.
If some of this request is exempt from release, please send those portions reasonably segregative & provide me with indexing, itemization & detailed justification concerning the information, which you are not releasing.
This request pertains to the years **** - ****. I hereby request a copy of all documents (meaning records, notes, electronic information, or any other information) described a follows; The Complete set of documents known as form 5546 pertaining to me & or a copy of the "AIMDIS print" used in lieu of Form 5546 according to your existing procedures & The complete set of documents known as the system of Treasury Dept./IRS 42.008, Audit Information Management Systems (AIMS) pertaining to me, & All information in my "Individual Master File" & my "Business Master File" & All documents which explain the purpose & use of the requested documents in paragraphs 1, 2 & 3 above & the symbols used thereon & or instructions on how & where to obtain such documents.
